Private Care Management  

Private Care Management is a program which provides case management services to clients over 18 years of age based upon a mutual agreement. The individual receives care management services to assist them in the assessment and coordination of their needs. Individuals who don't have the capacity to pay for such services will be evaluated for Medicaid benefits. Client may choose a service package or on going services tailored to meet one's individual needs.

Services Provided:

  • In-home visits to determine needed services
  • Help in the development of a unique plan of care
  • Information on resources so individuals are aware of all possible choices
  • Referrals to community agencies
  • Periodic review of individual needs
  • Education on insurance issues
  • Elder care employee assistance
  • Evaluation of current living situation
  • Caregiver counseling
  • Help with becoming Medicaid eligible
  • Assistance with out-of-state resources
  • Long-range planning
  • Ongoing monitoring of services and providers
