Citizenship and Identity  

A new law passed and became effective July 1, 2006 requiring people applying for and receiving Medicaid to provide proof of citizenship and identity. You MUST provide this information when you apply for Medicaid or when you complete your next redetermination form.

The following chart lists some of the types of paperwork that could be used for proof of citizenship and identity.

If you do not have any of the listed documents available or have any questions, please talk to your worker.

We will have to see proof of both citizenship and identity for each person you are applying for Medicaid and also for each person who is already receiving Medicaid. Please do not bring these documents until you receive a redetermination form or a letter asking you to bring in this information.

Original documents are required. Copies of documents are not acceptable.
