Persons Living with Major Mental Illness

Home and Community Based Services for persons with major mental illness (HCBS-MI) are provided in a home community setting to individuals, 18 years of age or older who would require nursing facility care without the provision of HCBS-MI. Individuals must have a major mental illness diagnosis as defined by Medicaid regulations to be considered for service.

  • Personal Emergency Response Home modification (limited to 10,000 lifetime maximum)

  • Homemaker

  • Non-medical Transportation

  • Personal Care

  • Respite Care

  • All Medicaid State Plan benefits

  • Case Management
