Long Term Home Health (LTHH)

Long Term Home Health (LTHH) services are provided to individuals who qualify for long-term care services under Medicaid ie. Home and Community Based Services (HCBS) who are 18 years of age or older who have a qualifying “skilled” care need. LTHH services are provided in the individual’s place of residence (home/assisted living), services are provided under a plan of care developed by the Home Health Agency and the individual and/or his/her family and services are provided on an intermittent basis.
