Child Welfare Records Request
How to Request Child Welfare Referral Reports:
Please note: The Child Welfare Assessment Investigating Case Manager has 60 days to finish assessments/interviews from the time the referral was taken and enter it into the computer system.
Child Welfare Referral Records can take 30 days after receiving the completed paperwork to complete the records request. Once the records are pulled, they are sent to the Mesa County Attorney Office for redaction. This may take up to an additional 2 weeks.
Former Foster Care Records can take up to 60 days to complete your records request, followed by an additional 30 days from the Mesa County Attorney’s Office for redaction.
To complete your request:
- Complete the Child Welfare Records Request Form (English) or Child Welfare Records Request Form (Spanish). Be specific as to what referral(s) you are looking for.
- Page 2 will need to be notarized. (There are notaries at DHS for your convenience)
- Attach a copy of your Identification/Driver's License
- Email the completed form, current phone number, and a copy of your ID to firstname.lastname@example.org
Once the Records Custodian has received the completed request form and copy of your ID, we will start your request and call you when your request is complete.
